Family Supportive Living Case Manager

Position Overview
At GAAMHA, we believe every person deserves access to the support they need, and that no family should face recovery alone. As a Family Supportive Living Case Manager (CM), you’ll walk alongside women and children in recovery, helping them stabilize, rebuild, and move forward—one family at a time.
You’ll provide holistic, compassionate case management services that reflect GAAMHA’s mission to redefine community support. Your work ensures that every family receives the tools, guidance, and opportunities they need to regain self-sufficiency, maintain lasting recovery, and strengthen healthy family dynamics.

What You'll Do
  • Collaborate with residents to develop and implement individualized family service plans that reflect their goals for recovery, stability, and growth.
  • Foster positive, supportive parenting environments by helping residents build the skills and confidence needed to nurture their children and maintain family cohesion.
  • Connect residents with community resources and mutual aid supports, including MAT/MOUD and recovery services that reflect their chosen pathway.
  • Support residents in the search for permanent, stable housing by coordinating with local housing authorities and housing assistance providers.
  • Work in partnership with community employment services to remove barriers and build career paths—connecting residents to pre-employment programs and job opportunities.
  • Coordinate transportation for residents to ensure access to critical services such as medical care, court appearances, recovery meetings, and interviews.
  • Administer urinalysis and breathalyzer screenings, promoting accountability and program compliance in a compassionate and respectful manner.
  • Maintain accurate records and case documentation, ensuring compliance with all agency policies and applicable privacy laws (HIPAA, 42 CFR Part 2).
  • Attend all required staff trainings, team meetings, and supervision sessions to ensure consistency and collaboration.
  • Serve as a community representative for GAAMHA’s Family Supportive Living program, building trusted relationships with local providers and partners.
  • Understand and enforce the requirements and procedures for reporting any suspected cases of child abuse and/or neglect in compliance with M.G.L. Chapter 119, Section 51A.
  • Perform other duties as assigned, always keeping GAAMHA’s mission and values at the forefront.

Qualifications
  • Bachelor’s degree in Psychology, Social Work, or related field and 1+ year of experience working with homeless families with substance use disorders.
    • OR LADC II certification, or 3–5 years of counseling/case management experience with similar populations.
  • Knowledge of multiple recovery pathways, including experience working with MAT/MOUD and other mutual aid programs.
  • Strong interpersonal, written, and verbal communication skills.
  • Demonstrated cultural competence and ability to support families from diverse backgrounds with empathy and respect.
  • Familiarity with electronic medical records (EMRs); KIPU experience preferred.
  • Ability to work both independently and collaboratively as part of a compassionate, mission-driven team.
  • Valid Massachusetts driver’s license required.
  • Must successfully complete background checks (CORI and all mandated screenings).

  • Supervision Given: None
  • Supervision Received: Administrative supervision from the Family Supportive Living Program Director; clinical supervision provided by the Clinical Supervisor.
