Job Summary
Provide training and supervision for assigned program participants in a professional manner, while increasing their quality of life and fostering more independent functioning. Instruct, coach, and supervise consumers in job skills training at a variety of community job sites and/or in Day Program facility. Ensure all work performed and the supervision administered is to the employer's satisfaction when assigned to a community-based job site. Provide a mature, adult role model for consumers regarding work habits, dress, interests, social skills, etc. Provide ongoing informal counseling/role modeling to the consumers as needed.  Provides outreach services and placement for volunteer and employment opportunities.


Job Responsibilities
  • Meet with each individual to individualized career training and placement strategy.
  • Support consumers in job sites in the community by facilitating their participation in assigned tasks; promoting communication and socialization with community members; setting up conditions for students to meet ISP goals at job sites and in the community.
  • Collect, graph and analyze data on consumer progress towards community goals and objectives.
  • Utilizes Set-Works Online Platform to create progress notes, billing, and scheduling appointments.
  • Creates Semi-Annual and Annual Reports for each client on the caseload.
  • Participates in the Annual I.S.P. review and occasional team meetings.
  • Transport consumers to and from home and adult program locations.
  • Identify internships and job placements.
  • Meet with program staff on a regular basis to ensure appropriate information sharing, coordination of program activities and service delivery.
  • Prepares and communicates job specifications and carefully matches potential applicants.
  • Refers appropriate participants to potential employers for consideration of the job openings and provide consistent follow-up to employers.
  • Interact with the employers to understand training requirements/job expectations/work quality/etc.
  • Professionally represent GAAMHA’s and their mission while on site at various places of employment.
  • Evaluate and track employment placement outcomes and retention for program participants.
  • Tracking potential employment sites and contacts.
  • Meets monthly budget/billing goals.
  • Other duties as assigned.

Qualifications:
High school diploma or equivalent preferred, unless required by state law, associate’s degree preferred, 1-3 years’ work experience, preferably with adults with disabilities preferred, basic fluency in oral and written English.  Candidate must also have an acceptable CORI (criminal record assessment) and must have a valid driver’s license and acceptable driving record upon hire and annually. 

Physical Requirements
Performing mechanical or physical lifts and/or transfers according to individual client protocols, bending, housecleaning duties, e.g., laundry, vacuuming, taking out garbage, standing, climbing, kneeling and pushing, e.g. wheelchairs.
